Course Overview

This course is designed for Care and Support staff across various settings, including Housing Support Teams providing direct care and support. 


It equips learners with the knowledge and skills needed to understand Person-Centred Support and how to effectively promote and implement person-centred approaches in their work.


The course also introduces a range of Person-Centred Planning (PCP) tools commonly used in adult social care to enhance individualised care planning.


This course aligns with current occupational standards, qualification frameworks, and the following key resources:

  • The Care Certificate Standard 5
  • NICE Guidance on Person-Centred Care
  • The Care Act 2014

Aims & Objectives

Duration: 3 hours


Criteria:

  • Understanding person-centred values and the purpose and content of individual plans
  • Assessing individual needs before creating a care plan
  • Using person-centred approaches to develop care plans with involvement from individuals and their families
  • Assessing an individual’s capacity to consent
  • Exploring care/support plans and person-centred tools
  • Roles and responsibilities in developing, implementing, and maintaining care plans
  • Linking care/support planning with risk assessment
  • Making Person-Centred Planning (PCP) inclusive for individuals with limited or alternative communication methods
  • Adapting actions and approaches in response to changing needs or preferences


Learning Objectives:

  • Explain person-centred values and describe the purpose and content of individual plans

  • Explain person-centred approaches to developing care plans with involvement from individuals and their families
  • Explain the importance of establishing consent when developing care plans
  • Describe the purpose of care/support plans and how to use person-centred tools

  • Explain the relationship between care/support planning and risk assessment
  • Describe how to make Person-Centred Planning (PCP) inclusive for individuals with limited or alternative communication methods
  • Explain appropriate actions and approaches in response to changing needs or preferences
