Course Overview
This course is aimed at Care and Support staff across different settings, including Housing Support Teams who provide direct care. Organisations have a responsibility under the Care Act to ensure that care and support plans are person-centred, promote individual well-being, and include detailed information to deliver effective care in a person’s living environment.
By focusing on detailed planning and consistent monitoring, staff can support each individual’s physical and mental well-being, promptly addressing any changes or concerns. Early identification and action help reduce reliance on other services and minimise the need for individuals to leave their homes for additional care.
The course provides learners with the knowledge and skills to accurately record, store, and share information. It highlights how person-centred thinking influences care provision and stresses the importance of precise documentation in supporting care plan updates. Learners will also explore their role in implementing person-centred approaches, with particular emphasis on effective recording practices.
